When does the blood splatter first appear on screen? What are the different levels of it? Does it change for hardcore?

The damage overlay spikes whenever the player is injured but fades rapidly if the player is still in good condition.
If the player is around 50% or so health, the bloody overlay will stick around until the player begins recovering.
If the soldier has less than 30 health, the overlay becomes almost opaque. The danger signs of an audible heartbeat and heavy breathing will sound and the blood will stay on the screen for some time until the player has recovered enough health.
Something like that.

To add onto Den's info if you look at the corners of your screen they will be a transparent red color until you are up to 100% and that will be signified with the distinct "tss" noise.
