It happens if you try to knife someone from too close. Try to stab from 1-2 meters away and you wont miss.

The poor netcode compounded with the way melee works leads to all kinds of... stupid.
When you press the Melee button, the animation starts. The Melee "projectile" does not actually come into existence until 0.5 seconds after starting the melee. The projectile reaches 1.75m from just behind where the camera is (something like the back of the player's neck).
The attacker shifts towards the enemy in ways that defy manual mobility... such as being pulled further into the air to stab an enemy a floor above you or stabbing an enemy through objects. In order to target the enemy, he must already be in/near the center of the screen. If the enemy is moving, the camera and attacker are driven towards him, it may even cause the player to pass directly through an enemy.
An attribution to the excess misses is that the redirection of the player screws with the collision detection.
The only thing I know to help a bit is don't move. Once you deploy the knife, don't manually walk/jump around or turn into/away from the enemy. Let the game take care of it. Unless it isn't guiding you, that is.
Stabbing enemies in the back or attacking crouching enemies - crouch and stab... or just shoot them. You'll swing at an oblivious "sniper" crouched on a rock four times to no avail and just shoot him in frustration like me.

I find continually walking towards my opponent works the best, but have no evidence to support it.

Walking directly forwards helps.
But NO strafing.

today i tried both "standing-stab" and "moving forward-stab"...they both fail the same...i suppose it's just a placebo there are 2 types of fail: -the enemy is not locked, the animation starts, u see his blood but the enemy stands still -the enemy is "locked" after the animation, u see the stab and the blood, but the enemy still stands... funny thing is that sometimes u kill the enemy with knife but there's no animation or "redirection"... i really can't get through it...i would like to know when and how the bug works, in order to avoid been knifed after i missed 2-3 times my killer

There's really no guarantee. The melee attack is just bad.
