The only thing I can think of is that that person was just about to throw a grenade when it hit him, so he threw it back.
I'll admit, I seriously lol'd myself when I saw it.

nah, that cant be right. Look at the bottom left and it shows that its his own nade, he didnt throw it back!

Grenades actually bounce off of people just like they would walls. I once tossed a grenade into a corner where someone was tactical loitering and it bounced off of him into the corner, against the wall, then back into him, then back into the wall, then back into him. My hit indicator went off like crazy until I got a kill resulting from the impact damage applying over and over and over again (I know it was the impact damage that killed, because the grenade didn't explode until after he was dead).

First grenade passes by him, his the rail and gets knocked down to the lower area. Second grenade is one that guy cooked himself, ready to throw at targets he could have already safely presumed would be in that area.
I've never seen the 'toss back grenade' prompt appear unless the grenade has already settled. It has definitely never appears as they pass over my head.

Look at the little kill icon in the bottom corner of the screen- it doesn't have the "______ is killed by (grenade icon) ________" text, it displays the suicide text, instead (meaning the player was killed by his own grenade). |
